️How to order your MultiSport Student Card  

Don’t have a school email address (e.g. @cvut.cz)? No problem! You can still easily order your card – just follow these simple steps👇 

Step 1: ️Fill out the online form 

Complete the Formstack form, available every month.

• Orders for the next month must be submitted by the 18th day of the month. 
• Orders submitted after the 18th will be processed in the following month.  👉 Open the Formstack form 

Step 2: Wait for an email with activation details 

Once you submit the form, within a few hours you’ll receive an email with your activation details. 
You’ll need them to create an account on the MyMultiSport portal. 

Step 3: Create your MyMultiSport account

Use the activation details to create your account on MyMultiSport. 
👉 Go to MyMultiSport 

Step 4: Order your card and activate it in the app 

After logging into the portal, simply order your MultiSport Student Card. 
Please complete your order no later than the 25th day of the month. 

Once your account is activated, click “Continue” – this will redirect you to the payment gateway. 
In the payment gateway, add your payment card – verification will be done by charging a small amount of 1 CZK. 

Then: 

• Download the MyMultiSport app 
• Log in using your new credentials 
• Open your card in the app – you’ll see a dynamic QR code which you’ll use to enter the sports facilities 
• The reception will scan your QR code 
• During your first visit or if you change your photo, you’ll need to show an ID for verification 

Step 5: Start moving!

Your card will automatically become active at the beginning of the following month. 
From then on, you can enjoy all the benefits of MultiSport – gyms, pools, climbing walls, yoga, and much more.
